Role Responsibilities:

1. Conduct assessments to determine individual and community needs.
2. Develop, coordinate, and implement individualized care plans.
3. Collaborate with other professionals, agencies, and stakeholders to best serve clients.
4. Provide counselling, therapy, and/or psychoeducation as needed.
5. Advocate for client rights and necessary resources.
6. Maintain accurate and up-to-date case files.

Qualifications:

1. A Bachelor’s or Master’s degree in Social Work or related field.
2. State licensure or certification where required.
3. Demonstrated ability to work with diverse populations.
4. Strong interpersonal and communication skills.
5. Ethical judgment and professionalism.
